General Care (Care Level 1)

Assisted living is a special combination of housing, personalized support services and health care designed to respond to the individual needs of those who need help with activities of daily living, but do not need the specialized care of a nursing home. Assisted living care promotes independence and dignity for each resident and encourages the involvement of the resident’s family, neighbors and friends; staff is available to meet scheduled and unscheduled needs.

  • Access to certified staff 24 hours a day
  • Three home-cooked meals with choice of entree, served in our elegantly decorated dining rooms
  • Dietician consultations for residents with special dietary needs
  • Regular monitoring of residents’ health and well-being
  • Monthly physician visits
  • Monthly blood pressure, weight, and nutritional status checks

  • Simple reminder service for scheduled meals, activities, and appointments

Higher Degree of Assistance (Care Level 2)

All services listed under General Care, plus…

  • Personalized assistance dressing, hygiene and grooming
  • Assistance with scheduled showers weekly



Progressive Care (Care Level 3)

This allows our residents to transition from the assisted living level of care into an area with a higher staff-to-resident ratio. Now residents can stay within their Sommerset Neighborhood longer as an alternative to prematurely transferring to a nursing home simply due to the requirement of more one-on-one assistance as health needs increase. The result is a higher quality of life for a longer period of time and reduced stress for residents and families.

All services listed under Care Levels 1 and 2, plus…

  • Assistance with incontinence care and toileting

Memory Care (Care Level 4)

At Sommerset Memory Care Neighborhood, your loved one will still be in a home-like setting. They’ll be safe and secure where their needs will be met. The Memory Care has a secured lovely outdoor area, allowing residents the opportunity to wander freely, inside and out, enjoying the simple pleasures of life without jeopardizing their safety. Floor plans offer cues that help residents navigate independently, so residents feel less confused as they dine and socialize with others. One of the best things about living at Sommerset Memory Care Neighborhood is that as your loved one’s needs increase, so does our assistance-without having to change their address or lifestyle. Our specially trained staff leads our residents through personalized therapeutic activities.  Meals are served 3 times a day, along with snacks. Our trained staff will assist residents as needed because good nutrition is a critical aspect of Alzheimer’s care, and our staff pays very close attention to our residents’ dietary requirements.
